Current copy of immunization certificate, signed/dated. [For the safety and well being of all individuals present in camp, we are unable to accept campers who are not immunized for any reason].
It is important that all forms and payment are sent at time of registration. Any missing forms will result in your daughter being placed on the waiting list until missing forms are received.
Registrations received without payment or a promissory note (for pending cookie dollars or camp credits only) will be returned. We cannot accept a promissory note for pending payment from troop. It is parent's responsibility to obtain those funds from the troop leader or treasurer and include with registration forms.
